Where can I find a consent for testing a minor?

If a minor, under the age of 18, comes to our centre's without being in the company of parents, we need the consent of one of the parents. If one of the parents is present at the visit, no consent from the other parent is required. Download the PDF below and fill in and bring to the visit.
